1. I fully intend to pick up my dog on the specified pick up date. If circumstances change I will notify Epic Blue Acres of the new pickup time ASAP and assume responsibility for any additional charges incurred. 2. Standard precautions will be used against the injury, escape or death of owner‘s pet(s). Epic Blue Acres Boarding will not be held responsible for injuries that occur, provided standard care and precautions have been followed as determined at the sole discretion of Epic Blue Acres. In no event shall Epic Blue Acres Boarding be liable for illnesses that arise during your pet(s) stay or after the pet has left the facility._________ (Initial)
We are committed to your pet(s) happiness when they are vacationing with us.
Dogs are taken on leashed walks. We promise to take every precaution to ensure your dog does not get loose during our outdoor sessions. However the risk of escape is possible. If you do not wish to take that risk, we can provide strict fenced in care for your dog(s) while here.

3.Epic Blue Acres Boarding is authorized by the owner to seek veterinary advice or care, including emergency care, at the owners expense. If we believe your pet is in need of veterinary care, if time permits we will make every effort to contact you before obtaining that care, but this document serves as our authorization to obtain life saving veterinary care for your pet regardless. The owner is responsible for expenses of veterinary care, whether or not you have been reached in advance. The owner agrees that Epic Blue Acres, in its discretion, may give medication or other attention when deemed necessary for the safety and health of the animal.

4. Owner agrees to pay the rate for boarding on the date the pet leaves our facility. If a check bounces there will be a $45.00 fee and interest will accrue at 10% per year. Any collection expenses, including attorney fees, shall be paid by the owner. Owner agrees the pet(s) will not leave our facility until all charges are paid by the owner.
CHARGES: Boarding charges are per calendar day, not 24 hour period. Any pet being picked up before 11 am will not be charged for that day. No charge days do not apply to holiday time stays. If you do not pick up your pet(s) during your pre arranged pick up window there will be a an "off time" charge. "Off Time" Charges: $15 for the first 15 minutes, $5 for each additional 10 minutes, from your scheduled time.Shuttle service (anyone from Epic Blue Acres Boarding picking up or dropping off your pet anywhere other than our facility) for your pet goes by mileage. _________ (Initial)

5. Cancellation Policy: Should it occur that you must cancel your reservations, please call us within 7 days of the stay. All deposits for reservations are non-refundable unless your reservation can be filled by another.If you do not call to cancel at least 7 days prior to drop off you forfeit your deposit. If you were allowed to make a reservation without a deposit and cancel a fee for the same amount will be owed.__________ (Initial)
6. I agree to present Epic Blue Acres with current vaccine records, before my pet boards. If I chose to bring records and present them at drop off, I understand if all vaccines are not current I may not be able to leave my pet for boarding. If my pet’s fecal or deworming has not been done in the past year, or negative was not the result of the last fecal test performed, Epic Blue Acres will collect a stool sample (for parasite testing) from my pet during boarding. I realize my pet will not be allowed to go for walks or play in fenced areas until results are in. I agree to pay a $35.00 charge for this and I will be presented with results of the fecal test at pick up. ________(Initial)
7. If my pet dies while in the custody of Epic Blue Acres Boarding, Epic Blue Acres will use reasonable efforts to notify me as soon as possible. Epic Blue Acres will take my pet’s body to my veterinarian if it is reasonably convenient for Epic Blue Acres to do so; otherwise, Epic Blue Acres may take my pet’s body to a veterinarian of choice. Owner agrees to pay any veterinarian expenses immediately upon request.

8. Owner covenants not to initiate legal proceedings of any sort against Epic Blue Acres Boarding as a result of injury, illness, death or damage to owners pet(s) arising out of the pet(s) stay at Epic Blue Acres and further agree that, should owner bring a legal action against Epic Blue Acres, the proper result of such legal action should be a summary dismissal in favor of Epic Blue Acres and owner further agrees to pay Epic Blue Acres costs, including but not limited to, reasonable attorney’s fees.

9. Owner agrees that by signing the “Authorization & Release” one time, it shall remain in full force and effect each and every time my pet(s) stay at Epic Blue Acres Boarding. _________ (Initial)
Owner hereby waives and releases Epic Blue Acres Boarding and it owners and employees from any and all liability for injury or damages resulting from their stay. I expressly assume the risk of any injury to my dog and/or cat including any and all medical expenses resulting from or relating to said injury, subject to the stipulations set forth in the authorization above. If I chose to leave personal items with my pet(s) Epic Blue Acres will not be held responsible for loss or damage of these items. If my pet(s) cause any damage to Epic Blue Acres property during his/her stay,I am responsable for the costs. I also understand if my pet is excessively destructive they may lose house privlages and be moved to a traditional kennel or crate.
I hereby agree to the forgoing as the owner of the aforementioned pet(s). Moreover, I certify to the accuracy of all information given about my pet(s) and that my dogs have not harmed or shown aggression or threatening behavior** toward any person. I also understand certain dogs may have to undergo a trial stay (overnight) before a long reservation can be made/approved.
